Open this file, find the line near the top of the <script> that says:
const API_URL = 'PASTE_YOUR_APPS_SCRIPT_WEB_APP_URL_HERE';
Replace it with the URL you got after deploying the Apps Script (Deploy ▸ New deployment ▸ Web app), save, and re-upload.
